Instructions for Submitting Your Grant Modification Form

NOTE: This form must be completed and signed by the organization President or CFO

1. Download the Grant Modification Form
   Begin by preparing your submission. Click CCN Grant Modification Form to download a copy of the application form.

2. Complete the Application
   The application consists of three sections that must be completed. Use the Previous and Next buttons to navigate between sections. Your responses will be submitted to CCN after you complete the final section, sign the form, and click Submit. A copy of your completed application will be sent to the email address you provide.

3. Save a Draft or Abandon Application
   Your application allows you to save a draft and resume later. If you need to step away, click the Save Draft button at the bottom of the form (to the right of the Next button). If you do not save a draft before exiting, you will be sent an abandoned application email with a link for you to resume.
